MIDDLE SCHOOL ATHLETIC PARTICIPATION REQUIREMENTS

NOTE: only 7th & 8th graders are required to have sports physicals and concussion forms on file and only needed if they are participating in sports.

NEW THIS YEAR!

Activity Registration for each sport opened on July 1 and will now be done through GoBound.  Parents/Guardians will now register through Bound and electronically upload copies of their concussion forms and physicals when they sign up for the different athletic programs Mount Vernon has to offer.

Students cannot practice or compete in activities until the concussion form and physicals are uploaded into Bound. Best practice is to register before school starts.

Students in fall sports must have a current Physical Form and Concussion Form uploaded to Bound PRIOR to the first day of practice. Again, parents should register at https://www.gobound.com/ia/schools/mountvernon and upload documents there. 

Students will be held out of practices and competitions if both forms have not been submitted online. Once the required forms have been submitted online, a student may participate in athletic programs. 

Fifth and sixth grade students are not required to have a physical form on file with the school until they begin to compete in school sponsored sports at the seventh grade level.
